Licensed Practical Nurse LPN
St. Martin, Located in Ashland, Ohio is looking to hire LPN's
The LPN will assume responsibility for the provision of patient care services based on plans of care established for each
patient. The LPN provides skilled nursing services under the RN Case Manager and authorized by the Attending
Physicians. The LPN will present and identify potential needs and resources for client care while administering the
highest quality of care; follow the standards of care per corporate philosophy and objectives; follow operations to
ensure safe and effective home care services to the community and present all findings to the RN Case Manager and/or
Director of Nursing.
The LPN is responsible for the following functions:
o Provides services in accordance with agency policies;
o Prepares clinical and progress notes;
o Takes vital signs and does complete physical assessment per skilled visit note and documents findings on said
note;
o Reports findings to the RN Case Manager after seeing patient;
o Assists the RN with carrying out the plan of care;
o Evaluates the effectiveness and outcomes of care;
o Participates in in-service education programs;
o Participates in regular case conferences and other activities to maintain communication between staff;
o Other job duties as needed relevant to the quality of patient care.
Hours would vary, Second shift and every other weekendEstimated Salary: $20 to $28 per hour based on qualifications.
